The Minnesota Department of Human Services (DHS) works to help seniors continue living in their homes and independently in the community for as long as possible. DHS develops policy and administers state and federal programs that provide home and community-based services, protective services, caregiver support and alternative housing arrangements.
Anyone may request an assessment for themselves or another person by contacting the local lead agency. The lead agency will determine program eligibility.
Please note that Elderly Waiver (EW), Alternative Care (AC), Community Alternatives for Disabled Individuals (CADI), CAC and BI programs have different application processes, financial eligibility requirements and covered services.
